Communication with Family: We may, using our best judgment, disclose to a family member, or another agency having custody of our client, any information relevant to his/her care.
Public Health: As required by law, we may disclose our client’s information to public health or legal authorities charged with preventing or controlling disease, injury, or disability.
Law Enforcement: We may disclose information to law enforcement as required by law or in response to a subpoena.
Disclosure for treatment, payment and health operations: We will share client information with other physicians and mental health professionals by providing various reports to assist in treatment. We will share information with State agencies which provide payment for services.

Maintain the privacy of our client’s records, and advise if a breach occurs that may have compromised the privacy or security of information.
